Few specialties challenge the buy-and-bill model the way oncology does. Infused and injectable therapies flow through hospitals, clinics, and physician offices that purchase the drug, administer it, and bill the payer for both the drug and the administration service. This is the most common model for provider-administered drugs such as injectables and infused therapies, especially in oncology and rheumatology.

Because the drug moves through the provider rather than a specialty pharmacy, buy and bill brands often have limited specialty pharmacy data. But limited data should never mean limited visibility.

What Buy and Bill Actually Means

Providers take on financial risk by purchasing drugs upfront and waiting for reimbursement, which may not always cover costs if claims are denied or delayed. Providers can also mark up the price of drugs, creating a potential revenue stream, but this comes with administrative burdens and inventory risk. Partly for these reasons, the model is becoming less common as payers shift to other models to control costs.

Compare that to brown bagging, where the patient obtains the medication from a specialty pharmacy and brings it to the provider for administration, or white bagging, where the specialty pharmacy ships the medication directly to the provider. In both of those models, the provider bills only for administration, not the drug itself.

The channel is genuinely more fragmented. Buy and bill manufacturers need visibility across specialty distributors, GPOs, and 3PLs, in addition to hub, copay, and other third-party data such as IOD, IDN, and GPO sources.

The absence of a specialty pharmacy doesn’t remove the need for coordination. Even without an SP network, care coordination is still required between FRMs, providers, and the hub to resolve patient cases.
